Safety Net Association of Primary Care Affiliated Providers of WNY
SNAPCAP is a membership association of 12 safety net primary care provider organizations working in partnership to provide the highest quality healthcare to the most vulnerable populations in Western New York. SNAPCAP Inc. is a 501 (c)(3) nonprofit organization that was incorporated in 2015.
SNAPCAP organizations work in partnership to strengthen the primary care safety net through collaborative projects, resource sharing, and collective advocacy in local reform efforts.
SNAPCAP occupies a unique niche in the Western New York healthcare system. Through a commitment to openly addressing and overcoming historical barriers to collaboration across traditional competitors, the organization has garnered tremendous potential to truly make an impact in care delivery for the most vulnerable residents of this region.
